Mandatory Washington State Disclosure

This agreement does not provide comprehensive health insurance coverage. It provides only the health care services specifically described.

VirtualMD is a direct-pay membership practice. Membership is not insurance and does not replace emergency care, hospital care, specialist care, surgery, imaging center fees, lab fees, prescription costs, medical equipment, or catastrophic medical coverage.

Scope limitations

VirtualMD provides virtual adult primary care within the designated scope of Dr. Patel. It does not offer hospitalization, major surgery, emergency care, specialist treatments, dialysis, physical rehabilitation, or procedures requiring anesthesia.

VirtualMD does not prescribe controlled drugs under DEA classifications II–V.

Insurance and billing

VirtualMD does not participate in private, state, or federal insurance billing for membership services. It does not issue superbills for membership care.

Patients are encouraged to maintain separate coverage for emergency, hospital, surgery, specialist, and catastrophic medical events.
